May be you have the file opened by Cross and the editor at the same time. It happened frequently to me.  That's why I often export the working mapping within Cross to have the latest working version in a safe place.

I recommend TextWrangler on OS X for XML editing and it works great. Color codes the text and identifies minor issues and notifies you (like missing parentheses). Sure beats using vi or vim.  (or notepad... :P ). Others have their own personal preferences.  Don't forget to share your final mapping with everyone here so others can use it as well.

With midiox / midipipe you can found some of the basic MIDI codes for any devices.
